Description
The Department of Veterans Affairs (VA) Bay Pines VA Healthcare System (VAHCS) has a requirement for deployment and sustainment of Automated Discharge Planning (ADP) Solution.
Veteran participation in his/her own discharge planning has shown to increase patient satisfaction and decrease re-admissions. Veterans who feel they had a say in their discharge planning are more likely to follow the plan. An automated discharge planning solution would allow Veterans to see pictures of each facility, read reviews and ratings, and know the location of placements. These options give Veterans the information needed to make discharge plans that are right for their individual needs.
The Contractor shall provide the ADP solution, which is inclusive of the perpetual license, software maintenance, helpdesk support, and training for Bay Pines VAHCS its connected sites. The solution is currently hosted within the VA Enterprise Cloud. The Contractor shall notify the Contracting Officer in writing if this situation were to change. The solution shall perform to the Service Level Agreement metrics noted within the Basic Agreement Performance Work Statement (PWS). The Contractor shall also provide implementation support of the solution and ensure that the referral network is maintained.
The Period of Performance for this action shall be 12 months with two 12-month option periods.
